How to Receive Services

In order to receive services from Partners in Housing Transition, you must first have a referral from any of the agencies listed.
Partners unfortunately cannot serve clients that have not come through a referral agency.

Community Healing Center

Provides inpatient substance support, outpatient services, AA/NA meetings, family counseling, or numbers of other services.

Healthy Babies Healthy Start

Works with expecting moms and families with children under 18 months. Participants must reside in one of the listed zip codes: 49001, 49002, 49006, 49007, 49024, 49048, 49080.

Home visits are required.

Kazoo County Ready 4s

Works with families of children aged 3 and 4 years old.

Healthy Families America

Participants can enroll at any time during pregnancy or before the baby is 3 months old, or if the child is under 2 years old and the family is working with Child Protective Services (CPS) or Prevention Services.

Family and Children Services

Programs are designed to provide the right forms of support at the right time for families and individuals who need resources. Our continuum of care includes crisis intervention services, counseling, home and community-based mental health, family preservation and parenting support, foster care, and foster care adoption services.

Urban Alliance

At Urban Alliance, our programs meet identified needs and promote sustained change and personal growth. Change of Status is a safe space for men experiencing negative behavior tendencies and substance abuse or those with a history of incarceration to embark on a 12-week journey designed to help them overcome the past and equip themselves for a different path forward.

Nurse-Family Partnership

Works with pregnant first-time moms within the first 28 weeks of pregnancy. The household must be eligible for WIC or Medicaid.

Kazoo County Drug Court

Caseworkers and probation officers send referrals for families involved in Women's, Men's, and Family Treatment Court.

KRESA Head Start

Children must be 3 or 4 years old by December 1st of the program year. Children who turn 3 or 4 years old by September 1 will be enrolled as soon as a spot is available, while children who turn 3 or 4 between September 1 and December 1 will be enrolled beginning September 1.

KPS Homeless Liaison Office

Provides rights and services to children and youth experiencing homelessness, which includes those who are: sharing the housing of others due to loss of housing, economic hardship, or a similar reason; staying in motels, trailer parks, or camp grounds due to lack of an adequate alternative; staying in shelters or transitional housing; or sleeping in cars, parks, abandoned buildings, substandard housing, or similar settings.

Clients have access to personalized case management, referrals (connections to community resources), material support, free pregnancy tests, parenting and pregnancy education classes, and limited transitional housing. All services are FREE and are either provided at our site (1441 South Westnedge), at a community location, virtual, or in the home.
